Admission

When admitting a client, the following is required:

  1. Passport or ID card
  2. document confirming disability (decision, opinion)
  3. statement from a family physician regarding the client’s suitability for a general-type social care institution
  4. statement from a family physician regarding previous illnesses and indications of medication use
  5. letter of guarantee for co-financing
  6. application from the client stating the desire to be in our institution
